More than a task list
The everyday things.
The things that matter.
Making a meal you enjoy. Getting to a favorite activity. Finding the words to ask for what you need. Your goals give support its purpose.
- Your preferences help guide the conversation.
- Your daily routines become opportunities to practice.
- Your family and chosen supports can be part of the plan.
- Your progress can be recognized one step at a time.
Getting started
A clear path forward.
01
Let’s talk.
Share what you’re looking for and ask about current service availability.
02
Make a plan together.
You, your support network and your case manager help shape the next steps.
03
Put your goals in motion.
Once services are authorized and arranged, begin working on the priorities in your plan.
A better understanding of support
What does person-centered support look like?
It begins with the parts of life you want to shape. A good planning conversation connects support to your choices, your strengths and your daily experience.
01
Listen to what matters
Bring your preferences, cultural traditions, communication style and familiar routines into the conversation. You can say what works for you and what does not.
02
Make the goal practical
Turn a broad goal into a step you can recognize: choosing a meal, organizing your day or preparing to meet someone new.
03
Keep the conversation open
Talk about what feels useful, what is changing and what you would like to try next. Your support plan is a place to return to as questions arise.
For individuals, families & case managers
Different questions.
One place to begin.
Looking for support for yourself? Start with a goal. Helping a family member? Bring their preferences into the conversation. Coordinating a referral? Start by asking about the service and location.
Ready to make a connection?
Use Guardian’s online referral form to begin, or call if you would rather talk through your questions first.
Refer a Client (opens in a new tab or window)
An inquiry starts a conversation; availability and authorization still need to be confirmed.
